Incident Reporting

91破解版 is dedicated to the protection of its employees and students. When injuries, illnesses, or incidents occur, we are prepared to immediately respond to the needs of the injured or ill.

  • Injury and Illness Prevention Program (IIPP)

    The prevention of occupational injuries and illnesses is of paramount importance at Western
    University of Health Sciences (91破解版). 91破解版 will maintain a health and safety program
    conforming to the best practices of organizations in our industry. Our objective is a health and
    safety program that will reduce injuries and illnesses and surpass the best experience of other
    operations similar to ours.

    Our Injury and Illness Prevention Program will emphasize injury and illness prevention on
    the part of both management and employees. 91破解版 promotes cooperation in all health and
    safety matters, not only between management and employees, but also between each employee
    and their co-workers. Only through a cooperative effort can an effective health and safety
    program, in the best interest of all, be established and preserved.

  • IIPP – COVID-19 Addendum

    This document is an Addendum to 91破解版鈥檚 IIPP and is intended to supplement that document
    as applicable. It addresses 91破解版鈥檚 measures to prevent the spread of COVID-19. It will
    remain in place until 91破解版 receives further direction from local, state and/or federal
    government officials and will be revised as needed.

    The following should be reported:

    • Near misses
    • Injuries
    • Illnesses
    • Exposures
    • Fatalities
    • Hazardous material spills

    In the case of an injury occurrence please complete the .

    Serious Injuries or Fatalities

    Supervisors and/or Campus Security must contact Risk Management and EH&S as soon as possible to report work related death, serious injury, or illness.

    OSHA defines an injury or illness as 鈥渟erious鈥 if it:

    • Requires inpatient hospitalization for a period in excess of 24 hours for other than medical observation
    • An employee suffers a loss of any member of the body
    • An employee suffers any serious degree of permanent disfigurement

    There are no reprisals or other job discrimination for expressing any concern, comment, suggestion or complaint about a health and safety related matter.
